These are what I use for my required 2nd notices to parents for IEP meetings. I keep a stack of them ready to go. When prepping for the day, I fill out the ones that need to be sent home and staple them into student journals or slip them into homework folders. Parents really like this because it's easy to read and they can put it on their refrigerator to help them remember! ;) Plus, it's easy documentation! Win-win!
